Insured patients: For patients with health insurance coverage, Chesapeake Urology will submit a claim on your behalf to insurance companies with which we are contracted (where we are considered a participating provider). Prior to your visit, we will also verify your member eligibility with your insurance company.
Referrals: Some insurance plans require you to have a referral from your primary care physician to see specialists such as a urologist. Patients are required to obtain referrals from the primary care physician office prior to scheduling an appointment with a Chesapeake Urology physician. Please check with your individual insurance plan to see if a referral is necessary.
Copayments, Coinsurance, Deductibles and Outstanding Balances:
- Copayments, or copays, are the fixed amount you pay for a healthcare service as determined by your individual health insurance plan.
- Coinsurance is your share of the cost of the service received. You pay a stated percentage of medical expenses after the deductible amount, if any, is satisfied.
- A deductible is the amount you pay before the insurance company starts to make payments for your covered medical services. Plans may have both individual and family deductibles.
- An outstanding balance on your account is the amount not covered by your insurance coverage and is your responsibility.
Patient Refunds: If you have overpaid your estimated patient responsibility (co-pay, coinsurance or deductible), Chesapeake Urology will process your refund and send you a check for a refund after the claim has been processed by your insurance company.
Self-Pay Patients: Patients who do not have insurance coverage or who desire services that are not covered by their insurance plan are considered self-pay. If you are self-pay, all payments must be made in full at the time of service unless other arrangements have been made with our patient accounting department. Our patient accounting liaisons are available to assist you with payments as well as work with you on payment assistance and options that best fit your needs.

Non-Covered Services: These are any services that are NOT covered by an individual's insurance plan. These services will be billed to you directly at the Chesapeake Urology self-pay rate. All self-pay services are billed and collected prior to any procedures or appointments.
Pre-authorization: Most insurance plans require a pre-authorization or approval for certain procedures, surgeries, a specialist referral or non-emergency health care services before it is allowed. Please understand that a preauthorization of a surgery or procedure is NOT a guarantee that your health insurer will pay for the service.
Diagnostic Testing: Your urologist may order blood work, tissue samples and other specimens to assist in the proper diagnosis and management of your urologic condition. While some lab work is performed at Chesapeake Urology's in-house pathology laboratory, you may be required to visit a third-party or outside lab such as Quest or LabCorp. It's important to understand that you will receive a separate bill from a third-party lab for specimens that are NOT processed through Chesapeake Urology's in-house pathology lab.
No-Show/Same Day Cancellation Policy: As a courtesy to our staff and other patients, we ask all patients to cancel appointments at least 24 business hours in advance of your scheduled service. Missed appointments and failure to cancel appointments within this time frame will result in a $25 missed appointment charge, which is not covered by insurance. For missed appointments at a Summit Ambulatory Surgical Center, a $100 missed appointment fee will be charged. Chesapeake Aesthetic Surgery and the Vasectomy Reversal Center of America have different cancellation policies. These Chesapeake Urology affiliates require two-week's notice for cancellations to avoid a $500 late cancellation fee.
Account Balances: Our office will provide you with monthly statements of all account activity and balances to help you stay on top of payments due as well as payments made on your account by your health plan. Account balances that are not paid in full when due will result in a late fee of $5.00 as provided by the Maryland State Law and may also result in the cancellation or rescheduling of appointments.
